REPORT OF BIRTH

REQUIREMENTS FOR REPORT OF BIRTH
(For infants/children born in Indonesia of Filipino parent/s)

Report of birth forms must be accomplished by the either of the Filipino parents or guardian. The infant/child need not be brought to the Embassy.

The following documents must be submitted in five (5) copies:

1. Duly accomplished Report of Birth Form (all original) accomplished by either parent;

2. Child’s registered Birth Certificate (in English or with translation in English) issued by the Civil Registrar of the Republic of Indonesia;

3. Copies of parents’ passports (data page only);

4. Copies of parents’ marriage contract (if applicable);

Notarial Fee for Report of Birth: Rp 325,000
