(08 March 2023) - The public is informed that in preparation for the transition of the Land Transportation Office (LTO) to the use of the new Land Transportation Management System (LTMS) in the issuance of Driver’s License Certification and History, the remaining supply of accountable forms (Annex A) will first be exhausted. Thereafter, the same template shall be adopted and printed in the form of the usual correspondence with the LTO letterhead and dry seal.

In this regard, the template used as the cover letter for Driver’s License Certification and History shall as well be changed from Annex B to Annex C. For reference of Filipinos who will need to secure the said document as well as its Apostille, the Annexes may be viewed through this link.
