REPLACEMENT OF A PASSPORT
The loss of a Philippine passport should be reported to the proper authorities as soon as possible. The grant of another passport to a person who has reported the latest issuance lost will only be given due course after a thorough evaluation of the applicant's qualifications and a careful determination of the facts surrounding the loss of the Government-issued passport. The passport is not considered lost if the booklet is in the physical possession of another individual/office/entity which is known to the bearer (example: passport used as collateral for a loan, passport confiscated by an office in relation to procedural requirements for a particular transaction/undertaking, passport in the possession of the employer/company, etc.). In cases like these, the applicant will be required to execute an affidavit explaining the circumstances of how and why the passport booklet was confiscated/kept by the individual/office concerned. The Philippine Embassy will confirm the details of the last passport issued to the applicant and require sufficient proof of Philippine identity before the processing of a replacement passport can be authorized. |
